Nurse Practitioner

Founded in 1975, Hospice by the Bay (HBTB) is California s first hospice and is the second in the nation. Our nonprofit currently serves children of all ages, adult patients and their families in the counties of Marin, San Francisco, San Mateo, Sonoma, and the cities of Napa, American Canyon and Vallejo. A pioneer in the field for more than 40 years, HBTB affiliated with UCSF in 2015.
Hospice by the Bay is currently seeking a Nurse Practitioner for our Marin area to perform comprehensive patient assessments, use evidence-based research data in clinical decision-making, establish and document appropriate diagnosis, create treatment plan and strategy of care based on the individual assessment; and provide expert guidance and education when working with clients, families and other members of the hospice team.
Essential Duties & Responsibilities (include but are not limited to):
  • Serve as the hospice patient s attending physician, as needed
  • Address the patient s needs, priorities of care, the need for collaborations when appropriate, and method by which treatment will be evaluated and the plan of action.
  • Provide and document expert guidance and education when working with the patient, the family and other members of the Interdisciplinary hospice team.
  • Participate in on-call duties as defined by the on-call policy. Coordinate call coverage, providing pertinent patient information with participating physician call group
  • Master s in clinical field of nursing or clinical field related to nursing.
  • Two years experience in an acute care, home health, hospice or palliative care setting preferred.
Certificates, Licenses, Registrations:
  • Advanced practice Registered Nurse with active Nurse Practitioner license.
  • Public Health Nursing certification highly desirable.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
